ABSTRACT

Objective:To investigate the fundus fluorescein angiography (FFA) characteristics of spontaneous regression in retinopathy of prematurity (ROP) and the range of retinal vascularization.Methods:A clinical retrospective study. A total of 82 eyes of 41 infants with ROP, who underwent FFA from January 2019 to December 2021 in Department of Ophthalmology of Xijing Hospital after completion of ROP regression, were included. There were 25 males (50 eyes) and 16 females (32 eyes). ROP was diagnosed in Zone Ⅱ in 44 eyes, with 38 eyes in stage 2 and 6 eyes in stage 3, and in zone Ⅲ in 38 eyes of stage 2. All patients underwent FFA examination under general anesthesia, at postmenstrual age of 70.70±12.25 weeks, after the natural regression of ROP was completed. Focus on the retinal vascular development, as well as choroid circulation and macular abnormalities, and compare and observe the differences between zone Ⅱ and Ⅲ after spontaneous regression. The extent of retinal vascularization was determined by the ratio between the distance of the center of the disc to the border of the vascularized zone (DB) and the center of the disc to the fovea distance (DF). The width of avascular area, recorded as the distance from the ora serrata to the vascular termination, was counted by disc diameters (DD). The measurement data between zone Ⅱ and zone Ⅲ ROP were compared by the independent sample t-test, and the count data were compared by χ2 test or Fisher exact probability test. Results:The linear choroidal pattern was present in 9 eyes (21.95%, 9/41), and the tortuous arteries in the posterior retina were detected in 32 eyes (39.02%, 32/82). It was noted that increased branching of vessels presented in 45 eyes (54.88%, 45/82), straight shape of vessels shown in 27 eyes (32.93%, 27/82), circumferential vessels arisen in 45 eyes (54.88%, 45/82), arteriovenous shunt appeared in 18 eyes (21.95%, 18/82), and capillary bed lost in 46 eyes (56.10%, 46/82) in areas from initial ridge to vascular termini. Punctate or linear dye leakage was observed in 23 eyes (28.05%, 23/82) during the late phase of FFA. Macular abnormalities, such as the absence of foveal avascular zone and hypoperfusion, were observed in 28 eyes (34.15%, 28/82), of which the macular ectopia presented in 1 eye. The mean DB/DF ratio of all the 82 eyes on the temporal side was 4.63±0.29 and 3.77±0.23 in the nasal. The mean avascular area on the temporal retina was 1.74±1.00 DD. Compared with ROP in zone Ⅲ, increased branching of vessels and dye leakage were more common ( χ2=9.303, 10.774; P=0.002, 0.001), the extent of temporal retinal vascularization was smaller ( t=-2.285, P=0.026), and the avascular area of the retina was more significant ( t=5.491, P<0.001) in zone Ⅱ ROP. Conclusions:Even after completion of spontaneous regression in ROP, incomplete retinal vascularization and vascular abnormalities may exist permanently in FFA, including those such as the tortuous arteries in the posterior retina, increased branching and straight shape of vessels, circumferential vessels, capillary bed lost and macular abnormality. Further appropriate follow-up is needed for long-term safety.

STUDY DESIGN: Case report. OBJECTIVES: We report 3 cases of loss of disc height after spontaneous regression of a herniated lumbar disc. SUMMARY OF LITERATURE REVIEW: Reports of spontaneous regression of a herniated lumbar disc were identified. MATERIALS AND METHODS: We conservatively treated 3 patients who were diagnosed with a herniated lumbar disc. During outpatient follow-up, radiating pain improved in all patients, but they complained of chronic lower back pain. Magnetic resonance imaging (MRI) was performed for diagnostic purposes. RESULTS: On MRI, spontaneous regression of the herniated lumbar discs was observed, but loss of disc height was also found. CONCLUSIONS: A herniated lumbar disc may be a risk factor for loss of disc height. It is important to recognize that a patient with a herniated lumbar disc can struggle with chronic lower back pain even if spontaneous regression of the herniated lumbar disc occurs.

ABSTRACT

Spontaneous regression of metastatic renal cell carcinoma (mRCC) was reported over the last century. However, there are no reports on spontaneous regression of mRCC by talc pleurodesis. A 43-year-old man who underwent left nephrectomy by RCC visited emergency room with headache and hallucination. Tumor was metastasized to brain, lung, and pleura accompanied by malignant pleural effusion. Talc pleurodesis by video-assisted thoracoscopic surgery was performed to treat malignant pleural effusion. After 7 months without specific chemotherapy, pulmonary lesions of mRCC gradually regressed. We thought that this phenomenon appears as an immunologic response of talc pleurodesis. We herein present a rare case of spontaneous regression of mRCC following talc pleurodesis. To the best of our knowledge, this is the first case of spontaneous regression in mRCC following talc pleurodesis.

ABSTRACT

Disc herniation is a common condition in the population and has a direct impact on the quality of life in patients, also causing functional limitations in the work place. Treatment protocols include conservative management and/or various surgical interventions. This paper reports on a case of symptomatic large extruded disc herniation, together with spontaneous clinical regression and total re-absorption. We show here conservative management, and a literature review of the main pathophysiological hypotheses for such an unusual evolution.


Hérnia discal é condição comum na população e tem impacto direto na qualidade de vida dos pacientes, assim como causa limitações funcionais em seu ambiente de trabalho. Protocolos de tratamento incluem manejo conservador e/ou diversos tipos de intervenção cirúrgica. Este artigo relata o caso de uma extensa hérnia extrusa sintomática, juntamente a sua regressão clínica espontânea e sua total reabsorção. Descrevemos também seu manejo conservador e uma revisão da literatura para a principal hipótese diagnóstica nesta evolução incomum.

BACKGROUND: There have been various reports in the literature regarding the conservative management of pituitary apoplexy, pituitary incidentalomas and Rathke cleft cysts (RCCs). However, to the best of our knowledge, spontaneous involution of cystic sellar mass has rarely been reported. We report 14 cases of cystic sellar masses with spontaneous involution. METHODS: A total of 14 patients with spontaneous regression of cystic sellar masses in our hospital were included. The median age was 35 years (range, 5–67), and 8 patients were male. Clinical symptoms, hormone study and MRI were evaluated for all patients. The initial MRI showed all 14 patients with RCCs. Eight patients were presented with sudden onset of headache, and 1 patient with dizziness. Another patient, a 5-year-old child, was presented with delayed growth. Three patients had no symptoms via regular medical work up. All 14 patients had no visual symptoms. The follow-up period ranged from 5.7 to 42.8 months, with the mean of 17.3 months. RESULTS: The mean initial tumor size was 1.29 cm³ (range, 0.05 to 3.23). After involution, the tumor size decreased to 0.23 cm³ (range, 0 to 0.68) without any treatments. Repeated MRI showed a spontaneous decrease in tumor volume by 78% (range, 34 to 99). The initial MRI showed that the tumor was in contact with the optic chiasm in 7 patients, while compressing on the optic chiasm in 3 patients. Five patients were initially treated with hormone replacement therapy due to hormone abnormality. After the follow-up period, only 2 patients needed a long-term hormone replacement therapy. CONCLUSION: The spontaneous involution of RCCs is not well quantified before. Their incidence has not been well demonstrated, but this phenomenon might be underreported. Conservative management can be a treatment option in some RCCs without visual symptoms, even in those that are large in size and in contact with the optic nerve via imaging study.

ABSTRACT

Cases of spontaneous regression of cerebral aneurysm remnant after incomplete surgical clipping have been rarely reported. This paper reports the regression of an aneurysm remnant after incomplete surgical clipping during postsurgical follow-up. A 50-year-old male presented with subarachnoid hemorrhage because of rupture of an anterior communicating artery aneurysm. An emergency clipping of the aneurysm was performed. A cerebral angiography, which was performed two weeks postoperatively, revealed an aneurysm remnant. The patient refused additional treatment and was discharged without apparent neurological deficit. One-year follow up cerebral angiography demonstrated a partially regressed aneurysm remnant.

Introducción: la fibromatosis agresiva es rara, benigna, con alto grado de agresividad y recidivante. Objetivo: caracterizar la fibromatosis en pacientes atendidos en el Hospital Pediátrico Provincial de Holguín. Método: se realizó un estudio, transversal y retrospectivo de los pacientes, en edad pediátrica, con diagnósticos de fibromatosis profunda que incluyó los años 2002-2012. Resultados: la prevalencia de la fibromatosis fue de 2,1 por cada 100 000 habitantes. Predominó el sexo femenino. La localización glútea fue más frecuente seguida del cuello y la pared abdominal. La etiología fue desconocida en el 75 %. La biopsia por aspiración con aguja fina no fue útil para el diagnóstico. El tratamiento inicial fue quirúrgico e incompleto. La regresión espontánea ocurrió en el 50 % de la recidiva. La quimioterapia y la radioterapia se emplearon sin resultados satisfactorios. Conclusiones: debe considerarse el tratamiento inicial expectante, pues la regresión espontánea se ve en muchos casos y la cirugía no previene la recidiva independientemente del grado de resección tumoral.


Introduction: aggressive fibromatosis is an uncommon, benign and recurrent disease. Objective: to characterize fibromatosis in patients attended al Provincial Pediatric Hospital of Holguín. Method: a retrospective cross-sectional study on aggressive fibromatosis in children younger than 19 years old was done during 2002 to 2012. Results: fibromatosis prevalence was 2.1 per 100 000 inhabitants. Females prevailed over males. The tumor was mainly localized in the gluteal region followed by the neck and the abdominal wall. The etiology was unknown in 75 % of cases. Fine needle aspiration biopsy was not useful for diagnosis. The initial treatment was 100 % surgical but incomplete; nevertheless, spontaneous regression of recurrence was seen in 50 % of cases. Chemotherapy and radiotherapy were applied to recurrence without successful results. Conclusions: the procedure, as the first line of treatment, should be considered because of tumor spontaneous regression. Surgical resection degree of the tumor does not prevent recurrence.

ABSTRACT

Merkel cell carcinoma is an aggressive and frequently lethal tumor of the elderly, associated with sun exposure and immunosuppression which is less common in the dark-skinned. We report the case of a 40-year-old woman who presented with multiple slowly progressive, mildly itchy ulcerated plaques of size ranging from 2 × 3 cm to 5 × 7 cm on the left knee of 1 year duration. Skin biopsy showed diffuse dermal infi ltration by small round cells with molding of cells and lymphocyte infi ltration. The cells stained positive for cytokeratin (CK) 20, CK7, neuron-specifi c enolase, and chromogranin. The skin lesions underwent spontaneous regression within 1 month of skin biopsy and have not recurred during the past 2 years. The immune mechanisms triggered by biopsy possibly explain the spontaneous regression.

Spontaneous Regression of a Big Subligamentous Extruded Disc Herniation: Case Report And Review of The Literature The most efficient method for the treatment of lumbar disc herniation is still controversial. The most important aspect is the application of the suitable conservative or surgical treatment to the right patient at the right time. In lumbar disc herniation patients, one must not precipitate except for cases that require surgical indications as in cauda equina syndrome, evolutive motor deficit and persistence of pain in spite of the narcotics. However, the spontaneous regression mechanism has not been completely determined yet. The proposed hypotheses are; dehydration, retraction of the disc to the hernia in the annulus fibrosis, enzymatic catabolism and phagocytosis. In this study, the case of a patient with huge lumbar disc hernia regressing by itself has been presented and the potential mechanisms of disc regression have been discussed.

ABSTRACT

Herniated nucleus pulposus (HNP) is a common disease that induces back pain and radicular pain. Some cases require surgical treatment due to persistent severe pain. However, in some cases, pain can be relieved with conservative treatment or at times relived spontaneously. Therefore, the most effective treatment method for HNP is undefinable. Spontaneous regression of HNP has been recognized with the advancement of radiological diagnostic tools and can explain the reason of spontaneous relief of symptoms without treatment.

ABSTRACT

Self-regression of Kaposi's sarcoma is rare with the exception of the iatrogenic immunosuppressive type. No cases of classic type Kaposi's sarcoma showing spontaneous regression have been reported in the Korean dermatologic literature. We report a case of classic type of Kaposi's sarcoma in a 69-year-old man which followed a clinical course of spontaneous regression. He presented with multiple asymptomatic beefy red papules and nodules scattered on both cheeks and upper extremities during a state of increase in the number of skin lesions at the initial visit. He had no history of immunosuppressive treatment and the laboratory findings were unremarkable. An anti-HIV antibody test was negative. A histopathologic examination revealed typical findings of Kaposi's sarcoma and positive staining with CD31 and D2-40. HHV-8 DNA was detected in a biopsy specimen. Without any specific treatment, the skin lesions healed without leaving any complications after 15 months, and did not show any evidence of recurrence during the ensuing 5 years.

ABSTRACT

Desmoid tumor is rare neoplasm characterized by clonal proliferation of myofibroblasts that do not metastasise, but often exhibit an infiltrative pattern and functional impairment. The etiology of this tumor is unknown, but hormonal, genetic, and physical factors play a role in its development and growth. The clinical behavior and natural history of desmoid tumors remains unpredictable and enigmatic. However, spontaneous regression of desmoid tumors is rare. Here we report spontaneous regression of an intraabdominal desmoid tumor in a patient who underwent total gastrectomy.

ABSTRACT

OBJECTIVE: This study was conducted to investigate the different cause of pelvicectasia and its clinical outcome. The most important management of pelvicectasia consist of the early diagnosis and evaluation of the pathologic abnormalities of congenital pelvicectasia. This will help to offer the guideline on management of neonatal pelvicectasia. METHODS: We examined one hundred and seventy-one live neonates who were hospitalized and diagnosed with pelvicectasia at Soonchunhyang University Cheonan Hospital from January 2008 to December 2008. A retrospective study was carried out in these patients for last three years. Renal ultrasonography was repeated at 1 month after birth and then 3 months interval. Diuretic renal scan with (99m)Tc-labeled diethylenetriamine pentaacetic acid augmented with furosemide and voiding cystourethrogram was done after 4 to 6 weeks of first renal ultrasonography. Patients were followed-up for 1 to 30 months (average, 7.2 months). RESULTS: Pelvicectasia was postnatally detected in 171 cases (33.7%) among 507 neonates. Males were twice than females. Additional imaging studies revealed that normal kidney structure was the most common postnatal diagnosis (97.1%), followed by ureteropelvic obstruction, vesicoureteral reflux, multicystic kidney, ureteric duplication. Spontaneous regression of pelvicectasia was revealed in 165 renal units (67.6%). CONCLUSION: There are many cause of spontaneous regression in mild to moderate pelvicectasia. Urinary tract infection occurs in many neonates with pelvicectasia. Mild to moderate neonatal pelvicectasia without vesicoureteral reflux is clinically much less significant. Accordingly, close observation with serial renal ultrasonography may be sufficient.

ABSTRACT

Although the spontaneous disappearance or decrease in size of a herniated disc is well known, that of a large extruded disc has rarely been reported. This paper reports a case of a spontaneous regression of a large lumbar disc extrusion. The disc regressed spontaneously with clinical improvement and was documented on a follow up MRI study 6 months later. The literature is reviewed and the possible mechanisms of spontaneous disc regression are discussed.

ABSTRACT

There have been many reports about lumbar synovial cysts, discussing surgical or interventional treatment methods. However, there have been few reports about spontaneous regression of the cyst. We report a case of a lumbar synovial cyst that regressed spontaneously. To the best of our knowledge, this is the first report of a spontaneously regressed lumbar synovial cyst in Korea. The patient was a 59-year-old man who presented with a 6-month history of insidious pain onset in the lower back and right buttock. Upon MRI of the lumbar spine, grade 1 spondylolisthesis was revealed at the L4-5 level. An about 6mm-sized extradural cyst was detected at the medial side of the right L4-5 facet joint. His symptoms were disappeared upon facet block with 0.5% bupivacaine bilaterally at the L4-5 level. Six years later, he revisited our clinic due to low back pain and left leg radiating pain. Upon lumbar MRI, aggravated L4-5 spondylolisthesis with left-side disc protrusion was observed. However, we could not find the synovial cyst around the right L4-5 facet joint that had existed 6 years prior. The patient was treated conservatively because he refused surgery.

ABSTRACT

Spontaneous regression of hepatocellular carcinoma (HCC) is extremely rare. We report a case of 67-year-old man having HBV-associated HCC with multiple lung metastases which regressed spontaneously. The patient had single liver mass and received surgical resection. The mass was confirmed as HCC histopathologically. Nine years after surgical resection, a 3.3 cm sized recurred HCC was detected on the resection margin in CT scan. Transarterial chemoembolization (TACE) was performed 3 times, and lung metastases developed thereafter. The patient received 2 more sessions of TACE, however, metastatic lung nodules were in progress very rapidly. We decided to stop TACE and followed the patient regularly without any anti-cancer treatment. Nine months after development of lung metastasis, the size and number of metastatic lung nodules decreased and were not detected anymore after 14 months. Serum alpha-fetoprotein levels also decreased to normal range and no viable tumor was noted in the liver. The patient is still alive 12 years after the first diagnosis of HCC and 16 months after lung metastasis developed.

Spontaneous regression is extremely rare in lung cancer and this in spite of its global high incidence. So far, less than 30 such cases have been reported in the literature. We report here on the case of a 68-year-old man who had the diagnosis of adenocarcinoma and in absence of any medical therapy, he had a partial spontaneous regression of tumor.

ABSTRACT

Lung cancer is one of the most prevalent cancers and it has the highest mortality of all forms of cancers. Although surgery, chemotherapy and radiotherapy are routinely used for the treatment of lung cancer treatment, little progress has been made in the treatment of this condition over the past 20 years. The histological subtype of squamous cell carcinoma (SCC) accounts for approximately 30% of all lung cancer patients. Spontaneous regression of non-small-cell lung cancer (NSCL) is an extremely rare phenomenon. Spontaneous regression of cancer (SR) is defined as a complete or partial, temporary or permanent disappearance of all or at least some the relevant parameters of soundly diagnosed malignant disease without any medical treatment or with treatment that is considered inadequate to produce the resulting regression.

ABSTRACT

The spontaneous regression of herniated cervical discs is not a well established phenomenon. However, we encountered the 3 cases of spontaneous regression of severe radiculopathic herniated cervical discs that were treated using a non-surgical method. Each of the patients were treated with a combination of manipulation, dry needling and analgesics. In each case, the symptoms improved within 12 months of treatment and magnetic resonance imaging (MRI) conducted at that time revealed marked regression of the herniated disc in all cases. These cases provide additional examples of spontaneous regression of herniated cervical discs documented by MRI following non-surgical treatment.
